The quest 'Brothers in Arms: Velen' suddenly completed *SPOILERS*
After the Ugly Baby quest, it suddendly "completed". I did spare Keira and go to Kaer Morhen but I haven't seen Letho lately in my playthrough. I did import with my 2% progress of TW2 save file

Unless it was the final autosave at the end of the game and you let Letho live then he will be dead in your playthrough. He is not needed for the Brothers in Arms quest.

I got the "Full Crew" achievement and didn't bring Letho. But where's Keira?

If she agreed to go to Kaer Morhen she will be there for the battle but not before.

Letho is not needed for the Full Crew achievement.
