Well first and foremost I would take a break from Witcher 3 altogether. I beat the game and have not played it since. That was like few years ago. NG+ is waiting for me.

You might do a silly 'just swords' run like me. Without using any signs or alchemy stuff(unless you absolutely have to, like yrden for nightwraiths or axii for alghouls). In death march, preferably.

Going to play it for second time(first play the GOG version around 150 hours) not played DLC so a lot will be new.

This. High Chaos run.

I've only run through witcher 3 once so haven't toyed with the various quests much.

I'm going to guess there are quests that are similar to skyrim in that there are alternative solutions to quests instead of picking one side or the other.

Depending on the parties involved, or if it's a subsequent playthrough, I'll experiment and try to find ways to screw over both parties/sides.

Go thru the game with the HUD OFF as much as I can, will do a different romance choice, try to make different choices while still sticking to what I want to do (in other words, not turn into obnoxious evil Geralt just because) and play with different ability upgrades.

Right now I'm working on Blood and Wine. After I will take a break I don't know for how long.
But when I start NG+ I already have planned what I want to do:
-Finish the game on hardest difficulty (from start to finish). I Started off the game on medium ended on hardest.
-Skip all the boring parts. No more exploring.
-In dialogues I'll pick the exact opposite of what I have.
-Last but not least, spend my money on wacky builds and play-styles.

I finished all the quests available in this game and tried to read descriptions of all characters to feel an amazing and real world. At least, it is the first time that I find the poor brother was killed by the old woman's husband in the quest "the fall of the house of reardon". Normally, I do this quest just for inviting Letho.

The most important target is that I decided to establish 3 different saves of "Be it ever so humble", then, I can go home to see Triss when I miss Triss; I can see Siri when I miss Ciri and so on. It is really comfortable and relax to see Triss and hear her tell you "Just a moment" while you say goodbye to her.

I'm not proud to admit that I tend to finish some games I really like multiple times but don't have the heart to make other decisions (unless I screwed up the first time). I remember when ME2 came out I finished it at least 5 times with only Paragon (good guy) choices.
Luckly, Witcher 3 is long enough that by the time you start again, the quests feel almost as fresh as the first time :steamhappy:
