I tried like 10 ways to fix this problem. Funny thing is your solution worked for me... Thanks man. I was searching like 2 hours to fix this. Btw I am using Sapphire Nitro+ RX 5700 XT.
If that workaround works then great, if not, no biggie, at least its clear now its a card specific bug of some sort

Not a single driver version has fixed the issue for me. This crash also happens around the Devil's pit.
This crash doesn't only happen in the Witcher 3, but these two locations in the Witcher 3 always, without fail, crash for me.
The only workaround i've found is to do these two locations with with the graphics turned to low.
Don't forget to report this is issue to AMD. Maybe one day they will do something about it. You can report this issue through Radeon software. Go to settings > report issue online.

Fixed a lot of problems for Navi card and RX 500 cards. I have RX 560X and I play this game on mix setting (mostly medium high with ultra texture/ssao) and 45+FPS average.
I solved this problem by setting the 30FPS max framerate, all other settings still "ultra"

I had the same problem with the card for several games. What fixed it for me was not changing the settings for the witcher. Instead I changed the globale graphics in the radeon software settings to "standard" and deactivated some other setting I do not currently remember. Then playing on "ultra" works just fine.
