How does the DLC work in base game?
I purchased toe GOTY edition which comes with the expansions. I choosed to start with basic storyline, thinking that the DLC ones were separate game modes.

However when I traveled to another map called Velen, the notification about Hearts of stone (that or Blood & wine, didn't remember quite) popped up and said I should play this when my character is min lvl 30. I'm currently only 3 lol.

So, do I just stick with the main quests and ignore the ones in expansions? Though I can't tell which belong to which unless perhaps the absurd lvl requirement gives it away.

You can savely ignore the B&W and HoS quests for now. They will wait until you are ready. You can even play them after the whole main story line. And many will recomend that.

As to how you recognize them: The quest markers for those quests are colour-coded for your convenience. Watch out for blue and orange quest markes. Those are the DLC markers.

Ignore the DLCs until at least lvl 35, don't rush anything, do your main quests, do side quests and run if you see a red skeleton head attached to any creature's health bar.
Well the red skeleton head is not really that bad if you're not on Death March, you can probably do max 5-10 lvls above your lvl depending on the creature and you oils/bombs/signs; just be really careful if on DM early game.

I personally completed the main game then the DLCs, HoS and B&W last, both in my first playthrough and NG+.

Thanks. It gets a bit messy after having so many quests, if only you could create an ignore folder.
