This also plays a role in the Blood and Wine DLC.

Why it's Yennefer of Vengerberg is an entirely different matter (though I'd say it's also used as a title, if not an official one.)
Despite his title, Geralt did not hail from the city of Rivia.
He is known as Geralt of Rivia simply because of his help with a battle on a bridge with The Rivia army or something like that. I dont remember the full details off the top of my head.
But that is indead the sole reason he is called Geralt of Rivia.
He was given that title because of his help in a battle. He isnt actually from Rivia. And really has no association with Rivia other than that battle.
The irony is, that before that battle he referred to himself as Geralt of Rivia. I think they made a point about it after the battle.
Vesimer had all the witchers he trained pick a surname. He wanted them to appear more trustworthy to potential clients.
Geralt initially wanted to have his name as Geralt Roger Eric du Haute-Bellegarde. Vesimir dismissed it as silly and pretentious. Geralt settled with "of Rivia" So Geralt of Rivia would be what he would introduce himself as to clients.
But the title of Geralt of Rivia is because he was given it by the queen for his help during that battle.
Its confusing.
I guess you could say he himself called himself Geralt of Rivia. And the queen legitimized it by giving him the title for his help with that battle.
Any who he is not known as of Rivia because he was born their or considers it his home or anything.
