B&W Orianna's quest is missed!!!

I have finished the DLC and I still didn't understand how the vampire trailer is connected to this DLC. Then I read more comments and I realized the lady was Orianna in the trailer but I don't really remember her from my gameplay maybe I met her once in the game (at the big meeting?) but no more time and I had no option to get the quest to Orianna. How is it possible? What I missed in the game??? Maybe I need to choose the Unseen Elder option or it is not the cause of problem? Is it really annoying.

You can miss the orphanage quest however if you don't choose the option to find Orianna. The first time I played through B&W I missed this quest as I thought (mistakenly) that finding Orianna committed me to taking the unseen elder route.

Yes, after completing Blood Simple you must choose your path. You can take the Beyond Hill and Dale route without doing the Blood Simple quest however.
