As you see Redania's Most Wanted failed, actually when started Reason of State, until that you can make Redania's Most Wanted whenever you want.
That option ONLY appears if you ignored Redania's Most Wanted, hence the game mark it as failed when Reason of State is given to you(providing you don't break Djisktra more than you broke him already).
Want another backup more. When Geralt is tasked by Radovid with finding Philippa there's no plain affirmative, the best Geralt will say to Radovid is something like "I will think over it", which means that by default Geralt is not that conviced in returning favor to Radovid the way he wants.
That is intended. The code of the game may fail, I'm aware of it's bugs, but this is not the case of this quests, you can choose to not do it.
Regarding failed quests, pulling out of memory:
Berengar's Blade: got the sword out of the quest. Is tricky because you need to find a ♥♥♥♥♥♥♥ moment to talk to Vesemir before he dies and with all that's going on around Kaer Morhen is not that easy, most of the time he was like "Hey, look at Uma" or "We are readying for the battle". In other runs I tried hard to find a moment to talk with him about the sword.
The gangs of Novigrad: entered the arena disguised as participant, picking the entrance from the guy killed near the entrance. This failed cause I choose to not go making a mess around places of Novigrad with Clever dwarfs.
Following the Thread: left for later when saw sailing Skellige was a requirement. Later ignored to finish it because was already over the recommended level.
Collect them'all: no gwent in 1st playthough, I'm not the only person that did this.
For Fame and Glory: courageous Skelligers died when they met an alghoul. Geralt can survive himself, but granting safety of weak ones is another story.
Brothers in Arms: Velen. Killed Keira, plain and simple.
I don't buy this, at least not from what a GAME should do. You having a quest in your diary doesn't mean you have to make it, the game never deems Geralt as bad if you decide to not act according the way you make Geralt speak, actually in some sense in the main quest Geralt may break his word when he agrees to work for Emhyr, motives aside and even if he wasn't jumping of joy he still agreed to work for Emhyr.
TW3 does actually good in this aspect, since it contemplates all scenarios and does not blame you for saying A and doing B, most will happen is natural reaction of the NPC that got aggrieved by Geralt blaming him.
Also you can say yes to Cleaver and hook off. When you take the quest Cleaver will say you to meet his men nearby the casino and the arena. If you don't meet them and do the things yourself the thing will be done your own way(did this too, so I know it happens). Ofc when you return to Cleaver after he will say you that he is paying you a ♥♥♥♥ because you didn't went with his dwarves.
