Witcher 3 is it worth doing all question marks.. ?
I am currently in the islands part of the game at 101 hours wondering if I should try to finish question marks before moving on ?

I usually do, but it's up to you. :) You can come across some side quests while doing them so that's why I think they're good to do. Plus I just love the game so more time in that world is never a bad thing.

I would say yes if your going for some of the other quests later on in the game. Certain Witcher sets can cost obsurd amounts of money if you do not get enough Dimeritium and etc. One quest (which most people wait until NG+ to do I believe) costs 15,000 alone. Unless your using money expoits I think its worth it.

One quest cost 5000 + 10000 + 15000. From the hearts of stone dlc

A lot of those '?' are just treasure locations where you find a sword or something that is not better then the ones you already have... or at least on the Island Skellige. On the B&W map however most of the '?' are connected to a quest. I am about to do the last main quest and I think I will just skip the '?' that are still left on the Skellige map. Doing the main and side quests already took me 130 or so hours :)

NG+ is the same game with just upgraded gear so for example Play 1 you can side with Yen and Play 2 you can side with Triss. Many other examples and allows you to get 2/3 endings. NG+ also allows you to keep your bombs, oils, your gear (i dont remember if the stash rolls over), and your money (I believe).

So basically after I do everything first run I can play again to try different outcomes and side with different people but with my gear interesting... I am currently on story and sword going to broken bones soon. Saving death march for second playthrough.

My second playthrough of this game will likely be some point in the future. (would play right away again great game and everything but I want to play other games in my library. trying to finish or at least play all games in my library for an hour or more.)

I explored all the ? on land, but in Skellige, all those in the water are smuggler's caches. So I did some for money, but not all. It's totally up to you. You won't miss anything if you don't do the ones out in the ocean.

Decided I will do rest of Skellige hidden treasures etc... now when would best time to go to Kaer Morhen be ???? Should I head straight their after the islands ( I did velen and novigrad already or do the heart and stone DLC first ??

The Skellige treasure marks are A HUGE waste of time as the "treasure" you find is usually worthless. It's akin to the time wasting "collection" items from the Assassin's Creed series. The Skellige treasures marks are perhaps the ♥♥♥♥♥♥♥♥♥ and lowest aspect of this otherwise, wonderful game.

I would not call a chance of getting Dimeritium or other valuable crafting materials as a waste of time. If you did not get most of the diagrams on Velen those pop up there as well.

Do main quest. Then hearts of stone. Then blood and wine. Hearts of stone is a really difficult dlc. So i recommend doing main quest first. But if you are not playing on death march, then go ahead and do it before the main quest. Either way is fine.
