Maybe do some rework around your combat options? I suppose I got lucky myself with how I changed those without understanding what I was doing. Just aiming for intuition and intuition is what I got.

My game loads in about 5 seconds now.
My issue is that I don't understand the flow. When I talk about Bloodborne and Dark Souls they have a specific flow, Bloodborne's flow is to move quickly forward and attack, Dark Souls is about dodging in cadence and rhythem. The Witcher 3's rhythem seems to be... run away when you can and try to wheel about it fight? I hate how combat in games gets devolved to "flow". When playing other games people can give tips to like dodge attacks and how to attack yourself but in this game it's just like "Nothing's wrong with it, you just need to git gud"
Geralt can roll and dodge.
Dodge to get out of the way of an attack and do a counter attack. Learn to read enemies so you can dodge just before they attack.
Use roll to put distance between you and an attacker. It's possible to roll towards an enemy, or to roll past an enemy, and to attack, but the roll is mostly used defensively.
A very simple move is to dodge backwards when an enemy attacks, and then use a fast attack. Most of the times you can follow up with another fast attack. Get the timing of the second attack right and you can attack a third time.
Dodging sideways a few times is a good way to get behind an enemy.
Parrying is only useful against human enemies and should be used sparingly.
Use Quen when you are facing multiple attackers, this will prevent you from getting staggered and getting mobbed after being staggered.
Use oils and potions when appropriate. For example: if you are fighting a bunch of wolves, use Beast Oil on your weapon and Thunderbolt. This will allow you to cut through a pack of wolves fast. Igni works well against beasts.
Axii (upgraded) works well against archers and monster that have a launch attack. Keep your distance, use Axii to stun, and attack.
OMG, yes. The camera makes me dizzy with lock on.
