I've played against every suggested character and innkeepers via quests' demands, including Dijkstra, Roche, Thaler, Lambert.
My deck is not complete; I know I am still missing several leader cards (since those show up in the interface) but unsure what other unit cards I have yet to obtain. I had long exhausted the number of cards I could win from "nameless" characters even before I was half way through the Skellige isles. The book's reported numbers confuses me as well since I did not actively pursue every single possible person in Skellige.
The achievement should've popped out when you complete the deck on the base game, there are others but no related to Hearts of Stone, instead the second expanssion, Blood and Wine (Skellige deck).
EDIT: Sells one of those dragon hunters and something else, you missing that?
