I started a new game for Blood and Wine with level 34 (recommended) but the game is way to difficult, even on the easy option. I played dark souls and bloodborne, but the witcher blood and wine with starting level 34 seams impossible. I am stuck at the witch in the cave below the house, 30 min after starting the dlc.

Just use yrden and aard. You can kill bruxa with one attack. Aard can take her to the ground and then you can stab her if you're fast enough. If you do not want to use this method, quen is option. Quen is actually too OP in this game.

Have you played the main game, because that ups the difficulty slowly throughout the game. On harder difficulties you should use the bestiary. At the beginning of the main game you get the chance to fail and not die. In the DLC's you just can't because your health regenerates slower (more vitality) and monsters do much more damage because of the higher level.

First of all keep quen up at all times to prevent taking damage. Secondly because of your Darksouls background you probably dodge with rolls. It's wrong in this game. You should use the Alt-button evasive movements as your primary dodging method as it lets you attack almost immediately and doesn't stop your stamina regeneration.

I'm not sure what potions/bombs you get if you start from one of the DLCs, but use Moon Dust if you have it. It's immensely helpful against vampires, especially the ones in the DLC. Yrden works too, as already said above, but I prefer moon dust since it's easy to accidentally knock enemies out of Yrden, whereas Moon Dust is simply a status effect (one that becomes permanent with the final upgrade for it)

I changed the roll and dodge buttons so with me space is dodge and alt is dodge. This makes sure that you dont roll all the time and mainly use dodges. On blood and broken bones i dont use quen that often because the second type of aard is great for managing lots of enemies at the same time. And with the ability with which you dont take damage while you dodge you dont take damage at all. But the main tip is: KEEP DODGING!!!

I disagree it kinda sucks even when upgraded unless you talk about alt quen then i no idea. Ursine set + alchemy more op imo

I am admittedly a piss poor player at this game and my NG+ strategy has always been overleveling waaaaay past the recommended level.

I never trusted suggested levels in this game. It's never meant anything. Remember Keria Metz's side quest? Recommended at level 5. You can easily get killed at level 10+ if you're under leveled.

Remember level requirements don't mean nearly as much as gear requirements. At level 34 you should have a full set of Witcher Gear. Don't attempt NG+ until you have one

It is all about opinions. For me, quen is too op. Especially with Aerondight. With increased sign intensity, you can take few more hits without any damage. Also, Invigoration enchantment is good. That is the build I use. Ofc, there are few more realy good. For example the ones with boosting toxicity and adding euphoria or whatever. But quen build is my favourite.

Whats this euphoria, I'd liek to make Geralt feel euphoria like I do sometimes in RL haha

When I get stuck, I scan YouTube for videos about that part of the game. Plenty of people have made walk through videos that demonstrate how to solve problems like this. For me it was the fight with Imelrith, damn was thart hard. I played that on Death March and after dying about 30 times I found a video on YouTube where a player demonstrated how to avoid him when he starts swinging his mace around and how to make sure you're in position to hit him quick once the mace is stuck in the ground.

Euphoria is that mutations you can learn in B&W. It is pretty good.

It is not that difficult for a BB or DS player. What truly makes this game impossible is upscaling, since it scales grouped agressive monsters to your level, breaking the entire intention of being in "group". Funny to fight a pack of wolves without a 0.1 second window to attack or see your bombs taking only 1/5 hp.
In blackmarsh you just don't need to get cocky when you'll do fine
