Redgill in Skellige is the most reliable place I've found. There's a group of 5 drowners along the coast - follow the path along the coast past the sign post and over the hill. Nearly always gives at least 1 lesser red, often more.

The best chance for drops are: Devourers, Alghouls, and the big Draconids or Vampires.
Next would be big bugs like Arachas.
After that, Nekkers (especially Warriors), Hags, and Ghouls.
I wouldn't farm Drowners for Red Mutagens.
But then again: I don't farm for anything in the game as eventually; everything drops.
Reds are supposed to be very rare. For good reason. I've always ended up biasing toward blues until I get enough reds (Though even a lesser red in a combat-mutagen slot is better than nothing).

What about fiends? For me, they are droping red mutagens. Devourers and Rotfiends mostly drop blue and green mutagens, but they are dropping monster bones everywhere :D Also alghouls are dropping red mutagens sometimes for me, but still rarely. And you can try Arachas. But I would go for fiends and they are pretty easy to kill too.

And also, if you have Blood and Wine, you can get one for free at the beginning. Also you can convert some of your green and blue mutagens to red. Since you can farm blue mutagens easily just by killing stupid drowners, that is probably the best way to get red mutagens.

But ♥♥♥♥ it, now I saw you wrote without DLC's. Damn, just keep killing the damn alghouls.

Yup. Mini-boss-like Monsters like Fiends drop reds as well.

Travel back and forth between Red Gill, Skellige for group of drowned dead and Toderas, Velen for devourers and alghoul is what I found to be the best.

Just don't worry about it. You will get them eventually.

If you really must, there is a farming spot on the mainland. It's a coast to the right of Benek. There are too ? at the top, and boat at the bottom. That stretch will spawn Drowned Dead, with a high chance of dropping red mutagens.

There's a coast on Skellige that's the same - The Whales Graveyard.

Are you trying to buff 3 red sections?

I don't farm them, but always manage to get 2 Greater Red mutagens by the end (ie I've found 18 lesser reds).

At the moment, I'm level 22, and have 2 Red Mutagens. You find loads in the Blood and Wine DLC, and you can transform 1 colour to another. Obviously doesn't help you with the main story though.
