On top of this, as a game designer I imagine you are never quite "done" with your project, but need to guard against things like feature creep, end up axing things or being forced to make less-then-ideal-choices, etc.
In the end, there comes a time you -need- to ship your product, however much you might wish you could polish it some more.
100% the reason why we get things like "enhanced" editions.
So, here we have CDPR giving us a pre-emptive apology in easter-egg format.
I think it's nice.
It seems like they're assuming you want to play again so you can make different choices, or "fix your mistakes".
I've only played through once, no NG+, and I had that letter also, so I doubt that's it. Since I only got the game earlier this year, I assumed the letter was refering to either fixing game breaking bugs, or the snafu with rebalancing equipment that left players unable to equip/use weapons and armor that they had already been using previous to the rebalancing.
That was my initial thought too. However, Why would CDPR apologise for choices you have made as the player?
I mean the letter is signed as being written from the Dev's perspective.

Then again, it's also totally possible we are all reading way too much into this and should just take it as a nice random little easter-egg.

I'd argue that the in-game, apology letter proves it was.
Ahh. Never had that bug. It worked fine in both my playthroughs.

We have raged and berated the games industry for dubious practices of all kinds you claim CDPR is also guilty of for years.
Doesn't seem to be really working does it?
And now, here we have here a game developer willing to admit to and right mistakes.
I say, maybe it makes sense and try and positively promote such behaviour in the hopes it catches on.
Further more: It was simply just a nice thing to do, nothing more.
No harm in saying it was just that: Nice.
Personally, I appreciate the effort.
