How to regen health on hardest difficulty?
Eating... Yeah, but food is expensive early in the game

potions or eating. loot and loot and loot plus you can eat raw meat you hunt or scavange after predators. eventually you'll find dumplings they're best deal (2 gold/pc).

There is also a skill you can equip that will heal you during the day time - slowly but it will save your food and drink.

There's a skill (Gourmet) that makes food/drink last 20 mins. If you are really struggling then invest a skill point in that and food will no longer be a problem. Oh and loot, loot and use swallow/white raffards as you only need alchohol to replenish those which you should get if you loot wherever possible. As you play you will also get better at dodgeing which will reduce the damage you take. After a while having enough food to heal is no longer a problem.

Sun and stars is a skill which u can unlock by levelling up , it regenerates health slowly when not in combat during day

I went with an Alchemy build this time around. The Troll decoction is very useful here.

Quen is best. Alternate mode will heal you, so you can just find some easy enemies and after few seconds you have full health.

my first play through death march all I had to do was abuse swallow, for whatever reason I never ran out of alcohol while refreshing potions during meditate

The OP is asking on the hardest level. Not sure there are such thing as "easy enemies". I'm playing on DM + leveled enemies and it is noteasy, haha.

I find that food is a lot more useful in this game, and since it has no weight, there is absolutely no reason not to loot everything. That Gourmet skill might be really good too (combined with food).

Swallow is probably nicer outside of a fight than within, since it interrupts if you get hurt (unless you get the Superior version, I believe). White Raffard is the best for within a fight.

Right now (lvl 21) my tactic during fights is food with some White Raffard if needed. But then I haven't really leveled up Quen. There are also some really good decoctions (Troll which I haven't, the one where spending Stamina gets you Vitality back, etc)
