Is the background story a sequel to nmrih1? Im a bit confused on the timeline.

check the guides for this game, one of them goes over the entire timeline, however yes this takes place after NMRiH1

in the first game you play as survivors during the unfolding of the outbreak, 2 takes place a bit after as CRC has become a bigger organization trying to help keep SOME semblance of civilization going.

There was a slight retcon to put NMRIH 1 into the 2020's instead of the 2010's like the original game, but as BRIMStone said you can assume than the first game takes place before and/or during the events of NRMIH 2 depending on the map in question.
