Where are grappling hooks?
Restarted a new game after the patches and went into the Black Hound Inn to purchase one to climb the wall and Pasca has none for sale. I have purchased them there before, did the patch change something?

I never saw them in Black Hound Inn but I did seem them (pre patch) in the Smith. Have you checked there?

Are you sure you got it from the innkeeper, not the blacksmith?

Ouch, you really should reload before you killed the whole Smithy, you get a pretty awesome shield from the quest there that´s kinda unique to the whole game.
Or did you loot the larder door afterward?

You get the larder door as a quest reward from the LIVE smith and you´re really not that far out to start from back there again. I´d really recomend it.
Remember, really good named items aren´t that common in the game and you can keep them up to date by upping the quality enchantement.
