Otherwise in base game there is also a perma death mechanic. You have two health bars basically, one is called endurance, one is called health. Endurance is like your immediate health bar, when it drops to zero you get knocked out.
Health is your overall health, which goes down very slowly when endurance goes down, and over enough time it reduces your maximum endurance. When health drops to zero the character dies permanently. Health typically can only go up by resting/camping though there's a few abilities that can increase it.
It's just a mechanic to wear and tear your characters to force resting every now and then.
The second game gets rid of the health/endurance system and gives you just health, but when it drops you get knocked out and an injury (which give various debuffs). Once you get 4 injuries you die permanently. Resting/camping heals injuries.
Just adding to what psychotron said, injuries are based on the type of damage that took that character down, like bruised ribs or burns or frostbite, with an associated penalty. You can also receive injuries from traps.
