What are we meant to do early game?
I've played divinity, some of poe2 (why im playing this game) and pathfinder etc.

Unsure what to be doing in this, just reached the first city and honestly it feels like a complete mess, named NPC's are 95% some touch the soul for no reason thing and if you get lucky you get a random quest to fetch or speak to someone.

I've been searching every area and building and find myself just constantly having to return to them because they were previously empty etc but later i found a quest to go in there in another part of the city.

Getting bored of it, any tips on what to do for it to be more enjoyable?

To advance the main quest, you need to go to first fires,

and look for the temple of Woedica, you will see a ghost, that will point you to another entrance. once you spoken to the lady, or get the info you need. You will get three new quests. then you need to choose a side. But it doesn't really matter too much on who you choose

The dozens, (HQ is in Copperlane)
knights of the crucible (first fires)
House Doemenel (Brackenbury)

be advised, that you can do the first quest from each faction, however doing the second quest for that faction will lock you into that faction, and you will not be able to do other quests for other factions of the city. So I would check online for what rewards you can get, and if it fits with your playstyle.

For example, the second quest for the Knights gives you a weapon that marks, which increases accuracy. the dozens second quest has a spear that you can enchant, probably one of the best options is to add marking on that. I'm not sure what House Doemenel gives in terms of rewards, as I never sided with them.

But there are quite a few quests to do in that city, and Ondra's Gift is a good place as well for quests, like the The Wailing Banshee (near the lighthouse to start that)

With the named NPCs, the gold nameplates are backer stories. You can safely avoid those if they don't interest you. If you do that, it should be a lot easier to navigate.

Most of the sidequests do stem from the main quest that ^ mentions, so if you're putting that off, you might just want to start there and then see where the branches lead you.

honestly not a fan of all the useless named npc's either. just hit 1,1 if you accidentally click on one

hmm i got an idea just now. maybe i'll try to do a playthrough where I kill each and every single last one of these guys...

They carry some pretty nice loot. Obsidian knew what's up.
If you do that, then you will need to kill also all other NPCs that turn hostile.

True. Though there are a few that are off on their own or inside buildings.
Let Eder turn Gilded Vale into a ghost town.

I wish he had an in-character reaction to that.

I'm wondering where I'm supposed to go and how to survive getting there. I'm playing a Cipher, and just got the wizard companion, but struggle to get anywhere. I'm still only level 2 and heading to the next region just a single wolf encounter took me down. I guess I expected to pick up more then one companion, or at least to get a front liner since the two of us are, well.... not frontliners.

After you stay at the inn and talk to the old woman's spirit in the tree, the man just south of the tree, Eder, will talk to you. He's a joinable fighter. Helpful hints that aren't in the tutorial, if a character has a voiced line on the map as you walk by, they're probably a companion, and companions are also shown on your map.

You can get to Caed Nua by going due east from Gilded Vale. Stay on the main roads in general to mostly avoid combat -- the game does kind of expect you'll eventually come back and clear areas out at higher levels. There's a cleric on the southeast road in Magran's Fork, which it sounds like you were probably close to finding before those wolves, and a tanky chanter at the walls of Caed Nua. Also now's as good a time as ever to hire an extra adventurer at the inn if you can afford it.
