Generally, I'm not aware of any achievement failing to work. Various people have unlocked all of them without problems since 2016 - even if playing offline.
Start with verifying your local installation files using the Steam app games library right-click menu.
Also, since you've unlocked achievements in 2015 and 2017, do you refer to a new playthrough or an old savegame you've continued with?
It's an old save that I continued playing.
If you've ever disabled achievements as outlined in the Wiki, that state gets stored in the savegame file. You cannot undo that.
I validated the files. All good.
Are you sure starting a new playthrough is the best answer here? I've put over 100 hours into this one. Can't you check my save file or something? Sounds like a bug on Steam's end to me.
For tech support, there's the official forum where you've posted already. Posting in multiple places at the same time about exactly the same topic usually is frowned upon.
A developer may be able to examine a savegame file uploaded to a public place and check internal state variables.
The Steam app is smart enough to unlock achievements even if playing offline and to transfer the data to your account once you switch to online mode again. No common issue with the achievements is known. Even at Google one only finds a very few topics and false positives.
I can forward you MULTIPLE instances of this happening to several different players. A quick Google search will show you that I am not the only this has happened to.
Bad customer service is also frowned upon ;)
Have you read through them all? Are they from early 2015 or after the v1, v2 and v3 patch series?

It is a public forum where everyone is free to give feedback. I've given you feedback as one of the regulars here since the 2015 pre-ordering days. In case I would have been late with my comment, somebody else would have confirmed that no common issue with the achievements is known. Rare cases would need to be examined first or have been troubleshot before. And as you can see, there has been helpful feedback about your console theories, too. With that confirmation that achievements work well with this game, make the best out of it. Upload a savegame - they are .ZIP archives under the hood - to some public place and post a link in the official tech support forum. It may take some time for a developer to take a look at it. Or search for more direct customer support channels via email or issue trackers.

Sound advice.
It's often considered rude because it IS inconsiderate (to other in the community) for someone to inflitrate forums with multiple posts off the same thing.
How is this an attitude problem *at all* in your eyes? Many many MANYdevelopers disapprove of doing exactly what you're doing. For someone to relay this info and you to get upset over something trivial, yet informative, is shallow.

In Obsidian forums you can report the issue and workers can help you much better than gamers here like said D´amarr. Here we are customers like you and is not members of Obsidian or community manager that move topics to official forums of devs. , etc.

I'm not using Steam overlay but that hasn't stopped other games.
But then again those are just archievements, so no biggy.
I figured out what I was doing wrong. I pinned the application icon to the taskbar and launched it that way. Apparently this bypasses Steam so it wasn't capturing my play time at all.
