They usually temporarily lower your prestige and/or security and sometimes even lower your reputation (permanently) with a city or faction if you don't pay them off or escort them.
They are not the same guests that want to speak with you about something.
Yes, they can be both types.
I wasn't aware that names of characters were re-used for other encounters. I've never seen that happen personally.
There is also another type of character encounter at the Keep, in addition to the Escort Missions & the face to face encounters. There are characters that show up seeking employment, which you can hire for defense. Those characters won't be seen unless you hire them, then they can be seen wandering around the keep, and during battles, though you can't enteract with them.
Edit: Nevermind, I need only have looked in my stronghold mission / prisoner list to see the options - the visitor has been taken care of, and all is well.
