How do you know about the quest, if it isn't even listed in the journal?
The first time you meet Sagani, you get the options to take a look at her figurine, tell her you're a watcher and learn about the first place where to travel to with her. I don't think you can soul-read anything at Pearlwood Bluff without the quest being active.
Maybe you've managed to break the game in weird ways. Sometimes blindly following walkthroughs makes strange bugs appear.
Is there a way to fix this, maybe with the console? I tried some quest commands but they don't seem to do anything.
So I checked the global variables and "b_Sagani_dead" was indead set to 1 somehow. I've read that there was some sort of similiar bug when the quest would fail as soon as you entered the Woodend Plains. This didn't happen for me but maybe it's a similiar root cause.
Since Sagani was now actually alive in my game I could start the quest with "StartQuest companion_qst_sagani" and it was finished already since all quest triggers worked the whole time.
So in the end I could fix this bug through the console thankfully. Maybe this is helpful for someone finding this thread in five years or so.
