Try this fix> Open console: ent_fire fall_trigger kill > proceed in bridge map transition
This should remove the trigger that checks for buggy vehicle. This is not a map issue is code issue, Valve fixed this after the orange box update way back, but they did not included the changes in the sdk code. MMod team seems it did not bother to play thru the entire game.
Upon closer look at the console, it looks like the same error that someone else posted about. Here's what happens in the console from the moment you step through into the transition to the moment it loads the previous save:

Maybe I entered it wrong? It didn't seem to have any effect.
Best is to contact the MMod devs and tell them to fix their code issue.
The save flip work-around works for now.
