While in warp look ahead of you for nebulas. Go around the nebulas - these will likely have ships or asteroids in them that will stop you.
You will still sometimes hit a asteroid field with no nebula (it's hard to see these coming) or ships just sitting in deep space. (you can't see these coming at all). You will just have to boost out and go to warp.
There is a bug that lets you go to warp easier though. As long as there isn't something directly in front of your ship that your ship would ram in to, press M to open the map (which makes the camera zoom out) then press M to close the map (which makes the camera zoom in) while the camera is zooming in you have a brief period to go to warp even if there's something nearby. Again this is a bug but it's a nice one.
When you get stopped by an object the message is "You have been inhibited by a stellar mass". When you get hit by a warp disruptor the message is "Warp drive Impaired" which causes the electrical bolts you jump across your ship. You can't go to warp until that wears off.
It's easier to escape from pirates if you have a fast ship and use the null grav booster. The null grav booster starts out slow, but speeds up the longer you hold the button down. It has a longer burn time than the other boosters and will let you outrun anything once it reaches top speed.
I can't think of anything else that might be pulling you out of warp.
