PS4 Controller issue
I've been playing this game with an Xbox one controller for a while but when I figured out the game has native ds4 support I wanted to try it out, but then this is where I got my problem. I plugged the controller in and the controller showed as an xbone controller, and on top of that the mappings were wrong (triggers were swapped with bumpers and cross and square mapped as X and Y and vice versa) if anyone has a fix for this please tell me I've been looking for hours.

You could use steams support for ps4 controllers, in those settings you can litterly change anything you want, swapping your triggers can be done easily.

how i do that, i have ps4 2016 controller, but it don't work or steam do not show as ps4 controller

You could try to run steam in beta mode, I am not sure if it's allready out of the beta branch. To do this go to settings > Account > beta participation > change and go into the beta.

Now connect your ps4 controller to your pc, go to big picture mode and here in the settings in big picture mode you should be able to adjust any settings of the ps4 controller you like.

I tried steam I just prefer having the ps4 mapping but yes I used it for my ac4 playthrough

Did you ever found a fix for this? I have the exact same issue for Unity, Syndicate and Far Cry 4.

I had the same issue.

FIX: Turn off Steam controller suport all together.
Steam -> Settings -> Controller -> General Controller Settings -> Untick EVERYTHING, regardless of whatever device you use.

Steam's non-Steam controller support seems to be pretty well borked at this stage, just don't use it.
