well, starting a new game doesn't give the achievement, neither on normal or hard or after finishing the game nor when you reset. On a different note, sometimes one can't scan enemies (especially Dogs and Trap) which is annoying when you need those for the database. Also it seems objects can't be scanned most of the time, I would assume I miss a lot of recordings even though I find and scan them but they don't track Game also shows different things at times. It didn't tell me how to destroy anomalies or...
