I believe you can also go to war on a civ who used to be your friend (and still has the diplomacy bonus). Let the alliance and/or friendship expire, declare a Surprise War, and see if it triggers.
Remember no Emergency is 100%. Once you meet the conditions the game rolls the dice to decide if one triggers.
That said, I've usually had no problems getting a Betrayal Emergency. To make it especially likely, put Cyrus in a game and try to meet him and make friends early on.
For example in the last game I finished, I had two CS emergencies pop up against the ai on back to back turns. I won both of them easily. I ended up killing 6 CS myself and 7/12 civs. I dropped dozens of nukes on the remaining players as I was prepping for Mars and yet not one civ accepted an emergency against me.
