This is just a guess, I did not try to go for the achievement yet.
So perhaps the order does matter. Try removing the head, and building it again or build a new one next to the holy site.
Another thing to possibly consider is whether the tile the head is built on needs to be owned by the same city as the holy site. Try swapping tiles if the head is being worked by another city
With your explanation, I appreciate how "tricky" this quest is. You have to build the Holy Site first, BUT you cannot built it just anywhere, as you might not be able to build a Colossal Head next to it. So, I have to hold off building the Holy Site unitl I meet La Venta and see the possible locations of the Colossal Heads. It is at that time that I can build the Holy Site, and then build the head next to it.
