30 turns seems to be a good time to wait to do maintenance. there still a risk but doesn't seem to be high, I got only one accident in more than 100 turns. What I been doing is using the congress to remind me to do maintenance. After I vote in the congress, I go to my cities with nuclear power and queue the maintenance project.
I had a nuclear accident after 6 turns :D
Not a meltdown but only venting of nuclear steam which did zero damage all told but it did focus my attention real quick on timely maintenance!
Basically, its RNG but the older the nuclear power station gets the bigger the chance of a serious incident.

Man it reminds of those civ 2 day, where I spammed nuclear plants to all my cities only to see the radioactive mess everywhere 20 turns later.
You started off as a brand new city at the beginning or early in the tech tree.

I did. Settled Scandinavia, which is realy good real estate if only hills and fish work. Only a Greece that randomly spawned in New Zealand could compare.
I managed to clear polution from Europe and Africa before I got bored though.
