No Production In Cities?!
My Level 10 City has 3 or 4 production. Is this a design issue?

What's the terrain like around your city? Sounds like all you have is grassland and wheat/rice fields. Those provide a lot of food, which helped get your city to 10, but it probably took you forever to build anything. When picking a site to build your city, make sure you build near or on hills, forests, copper, iron, or stone. You could also boost your city's production by starting a trade route to a city that has a harbor, encampment, trade district, and/or industrial zone (the more of them, the better).

Sending your traders to that city and having them boost production is your best bet. Try to get an industrial zone up so you have some minimal production, and consider clicking the "focus production" button to force your guys to work in that district. Build any mines you can.

Late game, you can plant forests and build sawmills on them to create production in the open spaces.

I also have this exact same issue in one of the city states I took over. When I conquered it I found out it had ALL food production and tiles. No production and all food. Honestly I was originally discouraged but decided it was a challenge to overcome. I opened up trade routes to slightly boost is production. I realized that not every city has to be buttoned up to perfection and accepted the challenge. It's forcing me to be a good city manager.
