same here. once walled.. almost impossible. i too have lost non walled cities. i have also lost a damaged walled city to a nuke followed by an attack, but only once. i have seen countless walled and non walled city states captured and actually look for the civs that do that. easy casus belli target and easy sue for peace without warmonger penalty.
the ai tends to denounce and pillage.. more than aggressively pursue human captures. at least in my experience.

I have never seen an AI using nukes, which is good I guess haha. I have one question, can city states actually capture cities? I mainly mean those city states focused on military.
i have seen city states take AI cities.. but only with my help. that being with my assistance, not actually paying to take over the city states units for 30 turns. in every case where the city state that i was suzerain of, once the city state captured the city.. it was immediatly razed. i have never seen a city state take an AI capital. not sure what would happen in that case, as capitals cannot be razed.
as for the nuke. yeah, it was quite surprising, not that Rome used the nuke, (nukes happen in my games from King and higher depending on how quick they tech), but that they actually attacked after they nuked. moving into fallout and taking damage to easily capture the city. pretty cool. i have also witnessed AI nuking each other. almost every turn hearing the nuke action sound can be get annoying, as they tend to spam them on the same city.
Same goes for the Devs obviously.
You should consider lowering the amount of period used in your text. CIV 6 is a great game but it has got it's flaws which suck, but mods do fix some problems!
