the stock earth map does not come with TSL (true starting location). If you set the game to culturally linked by ethnicity, then yes, they'll all start near each other coz they're all ethnically linked.
If you set it for random, then they'll all start close to each other without any ethnicity grouping, but still all clumped together within a minimum distance from each other.
you'll need to go find a mod if you want a real earth game, with true starting locations. I would suggest gedemon's yet not another map pack, find it on civfanatics
