11th Photo for Paz - Reminiscence Achievement not triggering

Don't wanna get into heavy story details so I won't spoil, but I gave 11th Photo, saw the explosion, Boss grabs butterfly, achievement doesn't pop. What am I missing?

Can you go back through Paz's door again or is it always locked now?

Its closed, has ha white sign on the door and it is locked. The photo is no longer on the wall and I woke up on the top of the Medical platform, you know that little platform with a door that doesn't open and you have to jump down

Yeah, if you have all that. you should have the achievement. If not, I'm going to say Game Glitch. There's a lot these days with the FOB cheaters jacking up other people's games. Maybe somehow you've been affected by something.
