Skull face is only part of BB imagination i think, its probable that Skull Face existed during GZ but is no longer the case in TPP, i thinik that BB creates an alter ego based on Skull face without even realizing that until the very end of the game story.
Probably skull face image is an easter egg of big boss being burned by snake(David) in metal gear 2: solid snake (msx) as we came to learn that his body was reconstructed from George Sears (Solidus) in MGS4, that's why he looks good at the end of that game. IDK i want to play this game really really bad -_-

Kojima might actually be the best troll indeed. He absolutely knew that everyone would only focus on the skull face aspect of the morphing sequence and he hid something right before skull face and everyone saw it but before they could question it he completely mind ♥♥♥♥♥♥ you and took your attention away from it by putting skull face right after.

What did everyone miss?
