Soooo.... not like that's a huge secret.
It's funny because I never wondered who the guy in the hospital was untill mission 46, but for a brain more in tune this would make sense as the player is to bealive he him self is big boss. And that must have been an illusion. That dose make the most sense as mission 46 shows he is infact real at some points at least.
Well as someone also puts it....it just....ends. Like serious. It just ends. No real "New Game+" but it just suddenly and dramatically...ends.
Plus you can see that in both Chapter 1 & 2. you can compare both chapters to a rollar coaster ride. The built up before the drop? That was all of chapter 1. As soon as that drop hits, that's all of chapter 2.
Sure chapter 2 had some great moments. But sadly it heavily relied on chapter 1. I've said in some form or another that. It would've made sense if Skull Face had been in Chapter 2 as well, shuffle around the story a little. Do some fine tuning. And then end the game with Skull Face and the Metal Gear boss battle. Everything else? the resident-evil style infection. The ceremony, Eli escape, etc. All of that could've come before. With some fine tuning and editiing, maybe adding a few extra missions could've helped.
THEN the game itself would've been ended properly. With the "Truth" mission unlock as an 100% condition. I've also said it myself, "If Skull Face is KIA. Who in the heck is the main villain for chapter 2? Eli? Oh wait his story can't be ended in MGS V because well...obvious and direct reasons".
This we know but we or I at least are wondering why he has no facial hair and has an eye. When designing a charecter model these are things that don't go un-noticed to the Kojima team.

Part of the illusion. A well-played illusion.
Like a fiddle,you might say
I agree 100%, which is why I think Chapter 2 is very inappropriatly named. I think "Chapter 1" is the full MGS 5 story as the villian is killed, and we get our salute into the sunset ending if Chapter 1 is what it is supposed to be reffered as. Chapter 2 is only what happens to the charecters, how Eli became Liqued, mantis, Psycho Mantis, and why we never get to see Quiet agian. How we go from liking the charismatic seemingly innocent Emeric and bealiving that he really had nothing to do with the destruction of mother base in 1974 but when he causes the parasite mutation and blames V for saving the rest of his men but in actuality fixing Emerics mistake, thus the end of Emeric. And of course the ending of Big Boss. Chapter 2 should be the "Postlog" as it should not be compared to the previous chapter in any way. Maybe this was an error in the localization team as naming Chapter 2 something different would have done wonders for the game. I do also wish that Skull Face lived on but that would defeat the purpose of the Main story starting and ending in the main story. It seems Chapter 2 only really existes because of the Shadow Moses incident but not for MGS 5 as it has much more to do with the former.
