"Her" Bandana, does it actually do much?
Or is it mostly for cosmetic reasons? I'm asking because it doesn't really look good on my avatar so I was thinking of not putting it on. But the item description says that it lowers the occurrence of serious injuries. But those are arleady really rare, right?

Yeah, mostly cosmetic.
you can upgrade it for infinite ammo

It's basically just a 50% chance of avoiding critical hits. Stuff still hurts, tho. So this bandana isn't as useful as the infinite one.
I love wearing the bandana, but from my experiene it actually makes you weaker, since critical injuries can give you a chance at refilling all your health+the slowmo to find cover and escape waht would normally be a situation where you're going to die.

I've died a few times but I've never once suffered a critical injury aside from the scripted one at the start. Not ever and I've maxed most of the things in the game (all sideops except the Quiet one, about 90% of the missions on S...).

I play FOBs a lot and since there is a chance of defender appearing I usually go the loud way from the start and spend majority of the time running in a rain of bullets. Never gotten a serious injury.

Does it even exist in the game?

Yes, I had to patch myself up a couple of times, but it doesn't happen that often.

I was going to comment that all the bionic medical augments+battle dress+bandana make you really strong,

But now thinking of it it kind of make sense so I guess the bandana would be good in FOB because going into that weak position with an army and the defender on your ass will most likely get you kill, but less useful in singleplayer cause it lower the chances of being able to use the magic resident evil spray.
Since i unlocked it i always wear it and i have to say i never gotten the press action button to cure myself anymore, i'm gonna start not wearing it when i do some sideops see how it goes
