Are you going on mad killing sprees all the time? You may have become 'demon', which involves his horn being bigger and blood on him at all times until you start doing heroic things to bring the meter down.

The blood becomes permanent when your demon score gets high enough and judging by that horn you passed that point.
It does. The script is working properly. You have been needlessly slaughtering soldiers and filling out the demon meter. So now you can't wash the blood off. Its not a glitch.
It does have "a proper script to take the blood off Snake when you shower". You've just killed too many people and made yourself Demon Snake (at which point your horn is biggest and blood doesn't wash off). That's actualy part of the game - kill too much and it shows on your body.
I think they are totally seperate, gaining heroism doesn't seem to reduce the demon score, if you can at all.
Heroism is different from the "demon meter". The demon meter doesn't show like the heroism one does.
Basically you cant wash the blood off until you do some "good" things.
