keep playing the missions to beat the game, but you can skip the extreme and subsistence ones. i did the first time through. now i really enjoy going into the super hard modes and testing my skills. its all optional tho.
if you have not seen the quiet subplot then get her and maximize her bond and keep playing. you will see a 2nd ending on mission 45. also the wandering mother base soldiers/medical subplot is crucial to understanding the game. those are optional side ops where you kind of search areas in the open world you dont normally go to.
Again, you have to do a couple of regular side ops too. Kaz will call you informing you of something happening at Mother Base, then the story will carry on. This is the way Chapter 2 works for the most part.
