Looking back I am surprized he didn't take a liking to Miller at some point. Maybe because The Boy doesn't want to take off a leg to symbolize it.
There is still one question though.
How about when he summoned whale to destroy the helicopter before ocelot come ? I don't think i saw he had those horn.
As for your question, I believe the whale was when The Man on Fire influenced The Third Boy.
Since he was chasing down Snake and Ishmael in the van and the whale came out of the water while he was chasing, this leads me to believe The Boy was influenced by The Man on Fire.
There is also the possibility that the whale influenced The Third Boy, but I doubt it wanted revenge on Snake or the guys in the firetrucks, or the XOF forces.

Oh. I just did some reading. A lot of people think the whale was summoned by Mantis when he was influenced by Snake to take out XOF forces. I would still say The Man on Fire though, since the whale was on fire. A lot of people are also talking about the Moby ♥♥♥♥ (novel) references in the game.
Also, I think I have been saying it wrong the whole time, it is actually The Third Child, but for the same reason. People seem to be using different terms in game and on forums as well.
