The game's rating system is not inmediate, it's commulative. Meaning you may get Octopus for a few rounds before finally getting a different rank.
I myself get Fox from time to time, Foxhound too, and there's been very weird situations in which, for example, I got the Butterfly codename without even bringing Quiet into the missions at hand. (I think I even got the Bat one once too, and I don't even know why, I haven't gotten every Codename once even, I'd say it's just all kinds of messed up).
btw. I killed quiet a few times with sniper rifles to get her back. Then I got some fox codename. I guess she just can't "spot" you. She can see you of course, but reflex mode (the "spot") never seems to be triggerd.
But this game's ranking system sucks.
This game wants to be a sandbox type game, but the ranking wants to force the players to play stealth and not use all the cool lethal weapons that you can get in the game.
