It's not outdated, you've got to finish the mission with S-Rank, finish all the optional objectives and then replay it again, for all the effects to kick in.
I've platinum'd the game (basically did everything there is to do) and everytime I replay any mission, just for the kicks, I get either S++ or S+ staff for the motherbase, which is already filled with them badboys. It also might depend on your MB level. Having overall C-Ranks wouldn't give you an S-Rank soldier in the end, you've got to fulton them in the first place.
Optional Objectives (means more GMP, more heroism etc.) - you don't have to finish them during the first time you're playing the mission, since you can replay the mission anytime you want. But remember this, your enemies will take precautionary measures the next time you're going to replay the mission you've already finished. Say, you've headshot every single enemy during your first take on the mission. Next time you're there, they'll be wearing helmets.
Meaning that instead of getting an A rank on Mission 1, you have to fulton 10 soldiers in mission 2 instead, for example.

If the option isn't there, you need to advance further into the story until it gets unlocked.
