Mission 40... beat Quiet again??
I was a bit surprised that I had to go through the same mission again...after doing several missions with her.

Yes, if you'd paid attention, half of the last ten missions are just replays of previous missions with harder modifiers. Mission 40 she'll one-shot you, shoots faster, and will dodge the boxes you drop on her head unless she's preoccupied.
