I guess you can't use a different save because cheaters used to modify their save files and stuff so Konami did something to this.
If you use the Delete Game Data on the initial screen when you first load up the game it is supposed to update the cloud Save with the new Data, however even in that case, it doesn't completely restart everything. For instance, anything in Gold will already be unlocked. The only true way to start from scratch is with an ALT steam acct. This is due to items in gold actually capable of being purchased and they didn't want to screw people out of their money. At least they were courteous in that regard.
If the Delete Game Data is good enough for your needs, but it still isn't updating the cloud then turn off Steam Cloud before clicking Delete Game Data. It will work once Steam cloud is disabled. You may enable Steam Cloud again AFTER you have completed the Hospital Scene, otherwise, it will copy in your old Cloud Save again. You must hit a checkpoint with Cloud Save turned off.

Wouldn't being offline would have done the same effect as deactivating cloud tho ?
Not particularly. I've done a lot of save file swaps when I was writing my auto-backup save game scripts and the restore backup script is ran entirely out of the game, which obviously you're offline if you're not even in the game, but still have to disable Steam Cloud or it will over-write your save file with the cloud file when you go back in, so I've always disabled cloud share before restoring a backup save file and enable it again after hitting a checkpoint. That seems to work 99.999%, Nothing is ever 100%.
However I had to delete/move some files in folder 287700 in order to get it to work and it did come with some other changes which I'm going to fiddle around trying to look into, like for example it also reset my steam hours.
Best guess right now is to just copy everything from folders 311340 and 287700, then either deleting all of them or deleting a few one in a trial and error fashion until you find which files need to be deleted or replaced to stop steam or the game from resetting them. The resetting still happened even with steam set to offline, however I never got a poppup about cloud saves (obviously).
The only files you should be concerning yourself with are TPP_GAME_DATA in the 311340 subdirectories and TPP_GAME_DATA1 in the 287700 directories. That is your save game file. Don't mess with the other files. 287700 is used in a purely offline manner 311340 is used when you go online. Otherwise, they're the exact same file just with different names.
Thank you for the clarification, I hadn't been sure about which files needed to be removed/changed so I just did them all.
You really only need to backup one of those files when making your backups. When you restore, just to make sure, just restore it to both locations and it will pick right up where you left off at. Just make sure it's name is appropriate for which directory it is in since the files are the exact same. The only thing different is the filename.
This is exactly what I do on my end, only I wrote my own automatic backup script with a Restore script to restore the backup to the appropriate filename in both locations. I only backup one of the files though. I have had a need to restore a couple times.
There is an exception though. The data regarding any development in gold (high level items) is kept online, so you can't actually delete that data, but you will still have to unlock it before you can use it. It won't show up until you've developed items lower than them.
