Kojima was on bad terms with KONAMI for a long time,which is clearly shown in MGSV4, the box with "no place for hideo"on it.
Moreover, KONAMI was determined to remove kojima's name from this game, (his name did disappear from the package of MGSV:TPP) you may take this as a rebellion of kojima against KONAMI.
His name appears at the beginning and ending of each mission because each mission is meant to look like an episode from a TV show.
He's keen on putting his name on what he creates because he's a poor programmer, poor manager, poor worker, poor everything. The only thing he's good at is watching movies. So the few games he created are his only diplomas. You bet he'd sign them. And it paid off, the bank accountant that lent him money for Death Stranding was an MGS fan. It's way better to have your name on the cover than having to explain you're some kind of part of the team that kinda did things for the game without actually programming or managing.
Even when people actually have egos, it's best to let them have it, let it flourish, maybe even play along and support it. People enjoy when others support their egos, that's true love right there.
