He, like 99,9% of every individual Venom interacted with, thought Venom was the real Big Boss.

I think the only ones in-game that suspected he wasn't the real one were only Huey - at the first meeting - and the AI Pod.

Likely not. He does call venom big boss but sarcastically toward the end which gives some evidence he know but it falls flat considering everything else so that sarcasm could really be goin taking a dig at the title of boss, someone who is ment to excel on yute battlefield, which venom didnt do since skullface managed to surround him

Motherbase soldiers do realise but only after truth, and the make comments too. Of course they are still with venom and will be even when outer heaven is formed

Miller and Ocelot were the only ones who knew the entire time. Zero as well, but he planned it to begin with

I would agree with you, but there's one thing that bothers me.

So Skull Face sent Quiet to kill Big Boss. In the hospital, we hear her say "not yet, the patient in the next bed saw my face". Then, she proceeds to try to kill you. This probably means she knew you aren't Big Boss. So after you meet her in Afghanistan, she probably also knows you aren't Big Boss. So if she does know, then i think Skull Face does as well?

She's sent to kill Venom. When she radios Skull Face, she's talking about Big Boss who IS the patient in the next bed. When this cutscene takes place, Venom is already wearing Big Boss' face openly, whereas BB himself is bandaged up.

What Quiet is saying is that she can't leave immediately after killing Venom because "the patient in the next bed saw my face". Then when she tumbles out of the window, the XOF troops are sent in to kill -everyone- rather than risk BB escaping.

I am thinking. Don't insult me. Venom Snake was lying on the ground, motionless, completely indisposed. Even if he's the target any sane would first take care of any other people in the room. Cipher could have fogured out which bed was occupied by Big Boss and instructed Quiet to kill the person in that bed.

You could also argue he calls em ''Big Boss'' thinking he's the real deal as a slight for the events of MGS3. Naked Snake was given that title almost as a participation trophy for betraying his mentor and ideals.

I feel like Skully saying that with sarcasm is a nice little stab at the legend.

Yes, but Big Boss has his face bandaged, so how could she have been able to tell that he had seen her? He was probably pretending to be still in a coma.

i mean wtf think about it:
you make a body double for a very important person is it even logical to send the real person to save his stupid body double?! and what for?!
the hospital mission is strange first of ishmael get hit like a bi** by quiet he doesnt have any blood when got hit by a knife, and it seems big boss is stupid enough to not bring even a pistol in a mission like that (:D), and worse than all he just vanishes and then returns several times
so if ishmael was the real big boss why he was there to save venom? i think when you are in 9 years of coma it took you more than 6months to recover and for big boss it took only 2 weeks according to the timeline?! and why ocelot himself didnt come to the hospital for saving venom?!
how you gonna explain the time paradox at the end:
ocelot pull ishmael from the ambulance and its still night then bring him by a bike give him a cigarrete and its day so we assume ocelot and big boss bid farewell to each other at the dawn
but then again ocelot went and save venom from the ambulance and its night!!!!!
so you wanna tell me that ocelot waited a whole day and by the night he came and svaed venom?! wtf?!
and even their plan with big boss and venom just sux imagine this :
venoms suppose to take big boss identity and big boss takes venom identity right?
but that means bug boss should go to plastic surgery too, so then he again went to another plastic surgery to recover his old face in outer heaven timeline?!
and to be honest here venom is a very bad body double he dont look like the big boss on the bike at all
strangely enough venom looks much older, has a face full of scars(still dunno how plastic surgery he went through didnt hide his scars lol) and he has a bionic arm so any idiot can see those are 2 different persons

By "the other patient" she meant the real Big Boss, Ishmael.

Yes. He was faking a coma at that time but she somehow knew it but stupid, arrogant Quiet, decided to take out the main target first 'cause she thought bandage guy Big Boss Ishmael wouldn't do a thing to save Big Boss Ahab - probably because he was pretending to be asleep, which may translate to cowardice.

In the end that whole ♥♥♥♥ is just a plot hole thanks to Kojima wasting Konami funds on ♥♥♥♥♥♥.
