Lots of fans recommend that you play the game series in release order rather than chronological order so it would be less confusing so once you're done with The Phantom Pain, you should start playing them by release order.
If you only bought The Phantom Pain, refund before you 2 hours and purchase The Definitive Edition so you get both Ground Zeroes and The Phantom Pain. Ground Zeroes explains why you're in the hospital.
A particular organization wants you dead. They want your dead because you are opposition to their goals, although they are not the primary focus of this particular game, so they're only mentioned in passing. For this game, it's well enough just to know of their existence and their desire to kill you.

That's like reading the last page of the book before you ready everything else. Play the game and you'll find out who he is at or near the end of the game.
He's from MGS3, although he was not a fire guy in MGS3. The fire guy was what he became after MGS3 or quite possibly as the result of the end of MGS3.
Because the game isn't about the soldiers, it's about you. Who cares otherwise.
He's your friend and ally, though he wasn't always. MGS3 he was your nemesis, now he's become your best friend, if one can actually have friends in the world of lies, deceit and espionage.
You haven't uncovered any plot holes yet. Just information that you want to know before it's time to know it or information that is only loosely significant to this game, but are actually references to previous games. If you bought the Definitive Edition which covers both games Ground Zeroes and The Phantom Pain, then you would get the rest of what you're missing.
To really understand most of your perceived plot holes though, you should play MGS3 and Peace Walker. That would cover pretty well all of the references that you're not getting from other games. Though they are all really insignificant to this game and don't really need to know them to enjoy this one.
Also why you're in a hospital and why the soldiers/assassin attack you is explained to you by the doctor. Try to actually listen and you'll follow the story. Also not everything is explained straight away at the begining of the game. It's the same for every game.
Complaigning about not understanding the story while picking the serie up at the 5th entry... Lmao.
5th entry? Are we counting MG1 & 2 and also PW? Wouldn't that make 5 actually the 7th or 8th in the series?
The game is called MGS 5 ( not really but you know.. ) And it's the guy's first MGS game, let's not confuses him even more.
• you in hospital because you were in a coma.
• soldiers want to kill you ordered by your enemy.
• the woman assassin was there to kill you, ordered by your enemy.
• the guy helping you is one of the guards that stationed there to protect you.
• the fire guy is a weaponized super soldier developed by your enemy.
• normally there are no water training for soldiers, only firearms and grenades.
• the cowboy is your aide, sent by someone who knows your past.
you are playing a game that is right in the middle of the whole series of story, those are not plot holes, those are the missing links that explains every plot holes in previous metal gear games.

PS: im only playing MGSV, so i'm not entirely correct. it is up to you to investigate further.
I don't understand how can someone start with game number 5 (and there is more MGS titles anyway) and be surprised that not everything is clear, especially in prologue.
I think it's that.
