Im just saying you don't understand the lore and be happy to help you, there are things in other games others might not understand just like you mentioned here, but you treat it as if it's on purpose and we can use that info to think about you like what whack a mole says:

is because in Ground Zeroes, your appache is losing control from when Paz jumped out of the helicopter because she *exploded*. With that in mind your helicopter backfires and spins in circles over and over to be t-boned by another appache or blackfoot helicopter (presumably an enemy helo based on MGSV context)
Then you wake up in the hospital and the introduction is explained by the doctor when he tells you about your coma and life status.
Some of you may actually know that the reason this "Snake" has bones and other fragments of body parts is because of the doctor with a custom identity given by you jumps in front of Snake to protect him from the shrapnel and explosion Paz gave out when she hopped out of the helicopter.
Those bones are from snake, soon- you realize that you were not Big Boss, but really the medic who supported him to be dubbed Venom Snake in MGSV because of his mass support to him

Freak accident, it's technically a shard of helicopter glass. Extracting it to analyze it would render him comatose, and he's singularly valuable to just about every single faction in the series.
It's a parable about how every conflict can rapidly spiral out of control, breaking all known laws of military science in the process. Very interesting to hear about WW1 militaries marching in line formation directly into machinegun fire.
