big boss in mgs 3 got his eyeball shot but in mgs v Ishmael big boss has two eyes maybe this is a mistake by the game devs lol

Big Boss' retina got burned by Ocelot's gun firing pretty close to his eye. In-universe, most likely he wore some kind of contact lense. In terms of the game, it's the ol' Kojima bait and switch.

When we learn Ishmael was the Big Boss, he was wearing the eye patch (during the meeting with Ocelot) but outside of it, how did you manage to see his eyes from the bandages XD

The face under the bandages is MSF medic from GZ. So you could say that Venom sees his old self helping him. "You're talking to yourself" - Big Boss talking to Big Boss or medic talks to his old self. Supposedly its all hallucinations (The way he sees Ishmael), but perhaps they just put a medic face so players coudnt tell for sure that its another BB. Or maybe they put medic on purpose.

That's the thing. Since everyone was after the Big Boss, they swapped the faces probably. Just to protect the Big Boss, since we were hypnotized at the beginning of the game as well.

During MGS3, after Snake gets shot in the eye and when you try to aim on first person view, it was clearly that you were using only 1 eye. I believe he lost his eye for good

You guys seem to misunderstand the concept of losing a bodily function vs not having said body part anymore.
A blind man still has his eyes. A deaf man still has his ears. A paraplegic still has his legs, for example.
Furthermore, did you ever think about how his eye was supposedly shot out, as you say? Wouldn't it leave an exit wound to the side of his head? If not exited, wouldn't the bullet nestle inside his brain? Would he even still be alive?

Not sure if you forgot that we are talking about a game, not a real life situation or you forgot that this is MGS. Both way, you can't expect realism.
