You can throw someone who is knocked out or dead by holding the reload button and picking them up, then hit punch to throw them back to the ground.
If MGSV, then you gotta hold the Action button while moving towards your opponent. If you stop your movement and then press and hold, you'll do a CQC hold instead of a CQC throw. It's important that you press and hold because quick presses will do punches instead of throws.
If MGO3, then it's similar to MGSV, except a quick press will always be a throw, while a press and hold will always be a grab.

This is a game that punishes stupid people. I've seen people get all sorts of pissed because they can't make good decisions.
