Cloaked in Silence never changed to [Replay], also never changes to [Reunion]

I have completed the game, lots of missions changed to [Replay], and from what I have gathered searching online 11 - Cloaked in Silence is also supposed to change to [Replay] on game completion.

I am trying to get Quiet back, but no matter how many times I complete the mission in a row without killing her [Reunion] never appears.

I completed the game before the 1.06 patch, could this be a bug?
If so, is there any other way I can trigger the mission title change?
I would really hate to have to replay the entire game without knowing if it will even work.

Don't kill her at all, not even once, that resets the counter

I have no mods, never used any.

I have also never killed her except maybe once when I first play the game, just to see what happend, this was also before the patch so the counter should not be affected.

Did the mission change name to get [Replay] before mission title after game completion for you guys?

You have to play it more than six times in a row (in my case, it was triggered at the eighth time). You can use lethal damage, as long as you don't kill her at the end cutscene.
Also, remember that "in a row" means completing the mission and starting it again right away. Don't go to any side op or even to Mother Base

I did complete the mission 8 times in a row as you say, as soon as I get back to the helicopter I start the mission again.

I will try again to see if it triggers at different number of playthoughs.

Defeat her non-lethally using a tranq sniper. Or drop boxes on her head.

Still no reunion, I have done this mission more times than there are missions in this game, every time I do them in a row and I don't kill her in cutscene...I even tried killing her to intentionally reset the count in case it was bugged somehow.

I also tried redoing mission 46, strangely enough I got Quiets weapon and outfit research back, but not Quiet, after I got the research I did mission 11 14 times in a row without killing her, but still no reunion.

The game is freshly installed and game integrity is verified.

Damn Konami for making this "replay 7 times" ♥♥♥♥♥♥♥♥ instead of something less time-consuming.

Something is definitely bugged about this, seems I am forced to delete my savedata and play the entire game over again if I want Quiet back legitimally, thank you Konami for buggy ♥♥♥♥♥♥♥♥ unlocks and serverside money on servers that still disconnect regularely this long after release.

I have played every Metal Gear game made except ps3/4 exclusives, and I have loved them all, and while I still love this game it is the only one that has given me this low level of quality.

Finally it worked:

After I redid mission 46 like stated in comment above, I got a sideops I had not before, "Quiet Exit", after finishing that mission, mission 11 fnally changed to [Replay] 11 - Cloaked in Silence...then after the 6th playthrough it changed to [Reunion] 11 - Cloaked in Silence, upon completion of that I finally got Quiet back.

Yay!

And I did use lethal weapons, I just did not kill her in cutscene.

Yeah, given that my game was reinstalled and verified etc...my only guess is corrupt savefile from cloud, unless I missed something vital during the missions...I'm just glad I can finally enjoy my game with my favorite support character :)
