AFAIR it ends @ mission 50 or so. The later episodes (and like half of Chapter 2 in general) were obviously supposed to look different and be proper missions, but Kojima run out of time, money and Konami patience. So if You're done I can link You 3 things You should probably watch to have full understanding. One of these is a missing game ending, "Kingdom of the Flies".
You mean the replay of the hospital mission? Where there actual boss was the man with the bandages?
So that's it? What happend to the girl with the gas mask and that ♥♥♥♥♥♥ kid that stole the robot? Is this literally the end? I'm only on 47% how can the story already end there? Is the other 53% side ops?

Sadly, yeah, "that's it". The game is unfinished. That's why I told you to report back when you're done, so I can show you the ending that didn't made it into the game while it definetly should.
Game completion is main story threads completion, side ops completion, tapes listened ratio and collectibles rate. The tapes contain plenty of valuable story material - listen to them dude.
That's a BOY in a gas mask. I think he goes on to become Psycho Mantis.
The kid was supposed to be the end boss fight, but they scrapped that mission. The cutscenes for that mission is still available online in an unfinished state, though.
Yup, that's the one. And yeah, you finished the game's main story, about as abrupt of an ending as it gets.
Unfortunately, the Eli subplot nevers gets any proper closure in-game, which is one of the points of criticism I have towards the game. If you want some closure on the Eli subplot, you could watch this: https://www.youtube.com/watch?v=LvsjeJJ5Ad4 About as close to an official ending as you'll get on that. I'm not really sure why they decided to leave the Eli subplot as is, I'm assuming the dev team were simply rushed by Konami to get the game out. Honestly, I kinda understand why this ending was cut (do we really need yet another battle against Sahelanthropus?) but I would've prefered any sort of closure over what we got in the end.
The remaining percentage consists of a few things, such as remaining side-ops and animal collection. I want to say optional mission objectives also count towards that percentage number, but don't quote me on that.
